Even if filled out online, all forms must be printed and mailed in to the TIC CAMP OFFICE, 4620 DITTMAR ROAD, ARLINGTON VA 22207 (or brought to camp), because we need valid dated signatures.
A VOLUNTARY DISCLOSURE STATEMENT is required for a criminal background check to be done on every employee of a children's camp. This form should be completed and sent in with an employee's signed contract. Print out and complete the two-page Voluntary Disclosure form.
A HEALTH HISTORY must be filled out annually for every staff member.Go to
"TIC Staff Health Form". A doctor's signature is not needed, but you must attach a copy of your immunization record, including the date of the last tetanus shot. If you don't have a copy, you can fill out this immunization form.
TAX FORMS must be filled out annually. All staff must complete the Federal W4 form. TIC-MD staff must also complete the Maryland MW507 form; TIC-VA staff must complete the Virginia VA4 form; and TIC- DC staff must complete the District of Columbia's D-4 form or a D-4A form, if you are a non-resident of DC. If you are a resident of Virginia working in Maryland or DC or any other combination, you must complete all state tax forms that apply.
NEW EMPLOYEES must prove citizenship or valid work credentials in order to be hired. Print page 3 of the I-9 form, complete the top part of the form, and bring it to camp along with valid identifcation (see page 4 to determine what forms to bring) where you will show it to a director, who will complete the bottom part of the form.
