Even if filled out online, all forms must be printed and MAILED in to the TIC CAMP OFFICE,
4620 DITTMAR ROAD, ARLINGTON VA 22207 (or brought to camp), because we need valid dated signatures. Faxes are unacceptable, because they are often unreadable, and the signatures must be original.

MEDICAL FORMS (three parts, mandatory):

(1) A HEALTH HISTORY must be filled out annually for every camper. Go to TIC Online Medical Form. (The health form will only be interactive if you have Acrobat Reader 7.0 or above. You may download a free copy of the latest version, Acrobat 9.0, by clicking here.) A doctor's signature is NOT needed. After completing the form online, print a copy to sign and mail in with forms (2) & (3) below.
(2) A photocopy of your CAMPER'S IMMUNIZATION RECORD must be attached to the Health History and must include the date of the last tetanus shot. If you don't have a copy of your child's immunization record (a form from the doctor's office or your child's school form), then you may print out and fill in by hand the one that TIC provides here: Immunization Record.
(3) Please also attach a photocopy of your HEALTH INSURANCE CARD , which would be required in any hospital emergency room.

SUPPLEMENTAL FORM

The two page CAMPER/PARENT INFORMATION form is not mandatory, but it is the very best way for your child's counselors to get acquainted with your requests and your child's needs and interests before camp. Please print out the two-page form, have your camper fill in his/her portion by hand, and then complete the parent portion, also by hand, and mail it to TIC.

If we don't have it on file at least two weeks before the session begins, don't bother — there won't be time for the staff to read it before they meet the campers. DO IT NOW, so you ensure that we are able to give our very best services to your child! GO TO Camper/Parent Info Form, print it out, fill it in, and mail to:
