A HEALTH HISTORY must be filled out annually for every camper. A doctor's signature is not needed, but you must attach a copy of your camper's immunization record, including the date of the last tetanus shot. Please also attach a photocopy of the health insurance cards (required in any hospital emergency room). The form is interactive online only if you have Adobe Acrobat 7.0 or above. We also require CAMPER/PARENT INFORMATION forms. The camper fills out one, and then the parent fills out the other. This informs your child's counselors of his/her needs and interests. We prefer handwritten forms, so you should print them out here, fill in, and mail as soon as possible. Go to "Parent/Camper Info Form" (2 pages).

If your child takes the TIC SHUTTLE between DC and Bethesda, we need a signed permission form. If you have not already received the form, print it out here, fill in, and mail as soon as possible. No child may ride the shuttle bus without this form. Go to "DC Shuttle Bus Form".

If your child is enrolled in TIC-MD for TENNIS, we need a signed permission form for riding the bus. If you have not already received the form, print it out here, fill in, and mail as soon as possible (or bring to camp the first day). No child may ride the shuttle bus without this form.
