Here we provide required forms which you should fill out, print, and send in by mail, fax, or email. (These forms will be interactive if you have Acrobat Reader 7.0 or above.) The completed forms must be received before June 1, 2012.

A
HEALTH HISTORY must be filled out annually for every camper. A doctor's signature is NOT needed.
A copy of your
CAMPER'S IMMUNIZATION RECORD must be attached to the Health History and must include the date of
the last tetanus shot. (You may already have a record from the doctor's office or from your child's school.
If you are a returning camper, we can use last year's form unless your child has had new shots.)
The CAMPER INFORMATION form is the very best way
for your child's counselors to get acquainted with your child's abilities and interests before camp.
The PARENT INFORMATION form gives you a chance to indicate any special
requests you have for your child.
